Patient- and Family-Centered Care From Qualified Nurses

At FWMC, we believe that patients and families are partners with our staff, working together to meet the needs of every patient. Our nurses develop an individualized approach to patient- and family-centered care, maximizing each patient’s health and healing potential.

Excellence in patient care is dependent upon the entire care team partnering together. Nurses approach to the planning, delivery, and evaluation of health is grounded in mutually beneficial partnerships among health care providers, patients, and families. Our commitment to patient- and family-centered care is a continual effort to be responsive to the needs and choices of each family.

In this approach, the degree of the family’s involvement in health care is determined by the patient, provided that he or she is able to do so. Our nurses work with both patients and their families to develop a care plan that includes all parties, giving control to the patient with a channel for input from their loved ones.
