The licensed Master Social Worker will be knowledgeable of and able to perform the following services: assessment, planning, intervention, evaluation, case management, information and referral, counseling, supervision, consultation, education, advocacy, community organization, and the development, implementation, and administration of policies, programs and activities.
|Education:||Master degree in social work from an accredited university|
|Experience:||One year of hospital social work experience. At least 3-5 years of discharged planning experience, and working with the geriatric population. Knowledge of medical terminology strongly preferred.|
|Licenseure:||Licensed to practice social work in the state of Maryland. If licensed LMSW, must be supervised by a community LICSW or LCSW if supervision hours have not been completed. Must show proof of supervision by a LICSW or LCSW|
|Skills:||o Ability to collaborate work with multiple people across the continuum spectrum of health care: MDs, RNs, ancillary services, patients, families, community vendors and other entities. o Ability to effectively communicate in a respectful, professional, and compassionate manner o Knowledgeable of post-acute care option, e.g. LTAC, Skilled or Subacute Rehabilitation, Home care, Community based services o Assess for, develop, and implement safe, timely post-acute care/discharge planning|
